I changed the output circuit stk 412-150 and it worked correctly but after 15 days I return with the fault that turns on but does not switch the relay and remove the STK and still does not switch the relay that is not stk problem

Check if the voltages in these transistors are as in the graph

I already check the voltages of trans Q 644 and in the collector there are 15v +
Q 647 THERE ARE 0.7 at the base
Q 666 THERE IS 0.7 at the base

Strange. If you have 0.7V on the basis of Q644 and 0.6V on the basis of Q647, the collector of Q644 goes zero by saturation causing the relay to close.
Check the transistors
